Tuberculosis Screening Questions

Tuberculosis, also known as TB, is a bacterial infection that attacks the lungs and sometimes other parts of the body. it is spread when someone infected with the disease coughs or sneezes and the bacteria is inhaled by others nearby.

If the answer to the following questions are NO, then no further testing or action is required. If the answer to any of the questions is YES, then additional follow-up may be required. If a TB assessment is needed, you will be responsible for the cost. As with any health information, your responses to these questions will be confidential and you will only be contacted if follow-up is needed.

You will not be allowed to enroll in classes until this questionnaire is completed.
